Comes in non-original packaging.  This plate is in fair cosmetic condition, with minor scuffs/scratches from prior use here and there.  Note several of the threaded holes have been bored out, and the clearance slots on the sides have been enlarged in a few places to admit larger mounting bolts.  A couple of additional holes have been drilled.
